CRB Guidelines for MU Members

  1. Disclosure forms and verification forms are available from Rockschool by contacting Josh Chelms on 0845 460 4747 or by email joshua "at" rockschool.co.uk.  A fee of £54 is payable at the time of request and cheques should be made payable to Rockschool Ltd and sent to Evergreen House, 2-4 King Street, Twickenham TW1 3RZ.  Please include a covering note with your full address and contact number.
     

  2. The disclosure form needs to be completed following the guidelines thoroughly.  Most importantly:

  • use black ink in BLOCK CAPITALS ONLY

  • write within the boxes

  • make sure all names are declared

  • ensure the full 5 year address history is provided in Section B. If you have not lived at this address for more than 5 years then please complete Section C.

  • section D must be left blank

  • section E might be signed and dated.

  1. The Veri-fy CRB3 form requires the applicant to provide three forms of ID   documentation, at least one of which must be photo ID such as a passport or photo driving licence.
     

  2. The applicant must take the CRB disclosure form, the Ver-ify form,  stamped addressed envelope and a fee of £5.11 to the Post Office where the identity checking and signature witnessing will take place. The form will be returned to Rockschool by the Post Office using the sae or Freepost envelope.
     

  3. Only original identification documents will be accepted for checking. See list for valid identity documents.

-England/ Wales/ Scotland/ Northern Ireland/ Isle of Man ; either photocard or paper ( a photo card is only valid if the individual presents it with the counterpart licence)

Birth Certificate (UK)
- issued within 12 months of date of birth – full or short form acceptable

HM Forces ID Card (UK)

Firearms Licence (UK)
